Picture archiving and communication system, popularly known as PACS is a comprehensive network of digital devices designed for image acquisition, transmission, display and management of diagnostic studies, workflow and reports.

The central storage device (archive) stores images.

The next step in the PACS workflow is the reading workstations. The reading workstation is where the radiologist reviews the patient's study and formulates the diagnosis. In this workstation all the functions like brightness and contrast adjustments, window leveling, measurements, calculation of HU values, magnification etc. are available for the radiologist and is as good as the radiologist sitting next to the console of the modalities. Normally reporting packages are incorporated in reading workstation that assists the radiologist for dictating the final report. Once the report is finalized, it is available for the referring physician to view with a click of button. Usually the report is ready with a digital signature of the radiologist.

The referring physician can view the images as well as report of his patients.

CD/DVD writing software are also available and are used to burn patient studies for distribution to patients or referring physicians. Filming option to print a hard copy from laser printers is also available. In many of the recent PACS, Radiology Information System (RIS) and Hospital Information System (HIS) are also integrated.

The PACS Server contains the image database. The clients connect to the server through a local area network (LAN) or a wide area network (WAN). Most of the modern PACS include web-based interfaces to utilize the internet for transferring Images.

These include faster delivery of medical images and convenience of viewing; ability to report as per free time from anywhere (reporting can be done from home for emergencies and in odd hours); reliable quality of images with flexible viewing and the ability to manipulate images on screen; instant access to previous examinations of the same patient for comparisons; better collaboration, as PACS can be viewed from multiple terminals and locations by a range of clinicians at the same time; improved learning and training opportunities, both for trainee radiologists as they develop their skills and for existing radiologists as there is more opportunity to collaborate with colleagues and share learning and experience; freeing up of valuable space within a hospital as storage rooms will no longer be needed for films; elimination of health and safety issues associated with chemical processing
